Please note that this page is intended for health professionals that wish to refer a patient for exercise in the gym. Individuals may join the gym directly by arranging an induction session with the Centre manager or gym staff.
The Cambrian Community Gym’s Exercise Referral Scheme is a great way of increasing the physical activity levels of people with specific health conditions. We provide tailored, supervised exercise sessions and our programme will help individuals to self-manage their condition(s) more effectively and with more confidence. We can also provide patients with information on additional services specific to their needs.

The referral form for this service can be accessed here. Our eligibility criteria are attached to the referral form. If you are unsure whether you or one of your patients is eligible, please call David Lemon on 020 8948 3351 (Option 2) to discuss.
We require a health professional to fill out and sign the referral form. It can then be printed out and sent by post or by secure email to: referrals@cambriancentre.org Once we have received the completed form, we will call the patient to arrange an appointment with one of our highly qualified staff.
Cost of Exercise Referral payable by the patient:
– Initial Assessment/Induction £12
– 12 subsequent sessions at £3.85 per session
Once the initial 6 sessions are complete, patients can then continue at the gym at our normal membership rates with the same level of care from staff (please see Gym page for membership options.
Please note that we are unable to accept repeated referrals for the same patient with the same medical conditions.
